How to Apply For A PhilHealth ID

PhilHealth (Philippine Health Insurance Corporation), is one of the Philippine organizations to provide support for employees in their health insurance. However, for them to verify that you are a member of the organization, you would need legitimate identification – your PhilHealth number, or your PhilHealth ID.

There are two (2) types of PhilHealth ID, The Regular PhilHealth ID, and the Digitized PhilHealth Insurance ID Card.

Yes, we have the classic laminated ID for PhilHealth; it is the one where you need a 2×2 photo. Now, PhilHealth is offering its members a digitized, PhilHealth ID card.

This new ID does not only serve as a valid ID. Of course, it proves your membership with the organization and at the same time, it makes you legally qualified to avail of its benefit services; one if which is life insurance. But first, you need to know how to get a PhilHealth ID.

So how do you get a PhilHealth ID card?


If you are currently employed, then you’re good because your company should be the one paying for your contributions.

On the other hand, if you’re unemployed or self-employed, you need to register yourself as an “Individually Paying Member” or a “Voluntary Member.”

Moreover, foreigners currently residing here in the Philippines are also part of this type. You would need to visit the nearest PhilHealth office and do the following:

Step 1: Duly accomplish two (2) copies of the PhilHealth Member Registration Form (PMRF)

Step 2: Submit PMRF to the nearest Local Health Insurance Office (LHIO) together with the following documents:

  • Two (2) 1×1 latest ID picture,
  • Two valid IDs; and
  • Any of the following documents:

For Uniformed personnel of AFP, PNP, BJMP, and BFP

  • A photocopy of General/Special or Bureau Order indicating effectivity date of retirement
  • Photocopy of Certification/Letter of Approval of Retirement from the GSIS indicating services of not less than 120 months
  • Statement of Services (photocopy) issued by a previous employer showing service of not less than 120 months

For GSIS Disability Pensioner / SSS Permanent Total Disability Pensioner before March 4, 1995

  • A photocopy of Death, Disability and Retirement (DDR) indicating the date of retirement and effectivity date of pension
  • Photocopy of Disability Pensioner Certification issued by SSS/GSIS indicating the effectivity date of pension or the period of coverage for the disabled pensioners.

For SSS pensioners

  • Photocopy of Death, Disability and Retirement (DDR) indicating the date of retirement and effectivity date of pension
  • Retiree/Pensioner Certification indicating the effectivity date of retirement (photocopy)

For GSIS pensioners

  • A photocopy of Certification/Letter of Approval of Retirement indicating the effectivity date of retirement
  • Photocopy of Service Record issued by the employer showing rendered services of not less than 120 months
  • Certification/Retirement Gratuity from the employer indicating services of not less than 120 months (photocopy)
  • Photocopy of retirement voucher issued by GSIS

SSS Survivorship Pensioner before March 4, 1995

  • Photocopy of Death, Disability and Retirement indicating the type of survivorship in nature and the effectivity date of pension
  • Photocopy of Survivorship Pensioner Certification indicating the effectivity date of pension

Other individuals who are not under the above-mentioned categories

  • Photocopy of official receipts of premium payments to PhilHealth
  • Any other documents indicating the months of premium payments to PhilHealth

HOW MUCH A DIGITIZED PHILHEALTH INSURANCE ID COST?

Payments for the PhilHealth ID

P90.00 – for Member
P358.00 – additional payment should you include your dependent

Spending an extra amount to get a digitized PhilHealth Insurance ID could be beneficial to all members together with dependents. Both could enjoy full access to PhilHealth’s basic services and perks that regular cardholders don’t have access to.

Ownership of the card is most useful to members who have maintenance medication since they can cut down the cost of medicine if they buy it from among the accredited pharmacy and enjoy services from accredited medical laboratories.

If you are going to register in any SM branch, you must present two (2) valid IDs. Also, if a member wants to declare a dependent, copies of IDs of the dependents are required too. Payment of P360.00 inclusive of one-quarter premium would be incurred for the registration fee and card. Bayad Center outlets, on the other hand, will charge P358.00 for the same card.
